Physical Therapist - Home Health

Shift: 8 hours/day

Contract: 14 weeks

We are seeking a licensed Physical Therapist to provide home-based physical therapy services to patients of all ages. The PT will evaluate patients, develop individualized Plans of Care, provide treatments, educate patients and families, and collaborate with physicians and interdisciplinary teams.

Perform patient evaluations and develop/update Plans of Care.

Provide therapy for neurological, neuromuscular, and orthopedic conditions.

Recommend and manage appropriate durable medical equipment.

Maintain accurate clinical documentation and follow infection-control standards.

Collaborate with physicians and other healthcare professionals.

Current state Physical Therapist license – Required.

Bachelor's degree in Physical Therapy from an APTA-approved program – Required.

1-2 years of acute care or rehabilitation experience – Required.

Home health experience – Preferred.

Ability to travel locally and perform physical activities including lifting, bending, pushing, pulling, standing, and walking.
