We are looking for a Licensed Practical Nurse (LPN) to support our growing Class A home care agency. The LPN will play a vital role in providing exceptional care to our clients, including medication management, wound care, oxygen therapy, and coordination with healthcare providers. The ideal candidate is a caring, detail-oriented professional who is passionate about working with older adults and ensuring they receive the highest standard of care.

Provide direct patient care, including medication administration, wound care, and oxygen therapy.

Assist in developing and updating personalized care plans based on the individual needs of clients.

Collaborate with Registered Nurses (RNs), caregivers, and healthcare providers to ensure the best possible outcomes for clients.

Monitor client conditions and report any changes in health or well-being to the supervising nurse or physician.

Educate clients and their families on proper care techniques, safety precautions, and the use of medical equipment.

Ensure compliance with state regulations and Home Instead policies.

Current LPN license in the state of Colorado.

At least 1-2 years of nursing experience, preferably in a home care or senior care setting.

Compassionate and patient-centered approach to care.

Proficiency in using electronic health records (EHR) systems.
