We have an exciting full-time career opportunity for a caring, outgoing, organized, and detail-oriented Social Worker at Edenbrook Lakeside, a welcoming skilled nursing facility!

Responsibilities In this role, the Social Worker will support nursing home residents on an as-needed basis by promoting their overall well-being and addressing emotional and psychosocial needs. This includes conducting interviews and assessments to identify individual needs, developing resident-centered care plans, and collaborating with staff to implement and sustain interventions that enhance quality of life. The Social Worker will also provide coverage and assistance as scheduled or when needed.

Duties Provide Social Services oversight, completing duties as assigned for the residents of Eden Senior Care.

Work closely with the Marketing Director to coordinate the referral-to-admission process.

Communicate with insurance companies to obtain prior authorization for admissions; Assist new residents and family members with the completion of the Admission Paperwork.

Provide information to residents/families regarding Medicare, Medicaid, and other financial assistance programs available to the resident; assist with completion of Medicaid applications as deemed necessary.

Participate in resident assessment - provide background information and social history, as identified in the MDS process, in accordance with the RAI manual.

Involve the resident and family members in planning objectives and goals for the resident; develop a written plan of care to work towards meeting identified goals.

Complete timely progress notes on each resident.

Have an open-door policy to all residents, family members, and staff to listen to their problems or needs; Serve as an advocate for the resident and his/her rights, helping the resident learn assertiveness in communicating their needs, wants, and problems to the appropriate departments and agencies.

report to IDT about pay changes, care conferences, concerns, etc.

Initiate and lead the discharge planning process; coordinate with outside resources such as DME, Home Health, Hospice, etc., to ensure a safe transition to home or lower care setting.

Assist in providing in-service education and orientation for employees regarding Resident Rights and the social service needs of residents.

Bachelor's degree in Social Work or a Bachelor's degree in a Human Services field, including but not limited to Social Services, Sociology, Special Education, Rehabilitation Counseling, and Psychology.

Preferably at least one year of experience working as a Social Worker in a Skilled Nursing Facility.

One year of supervised social work experience in a health care setting, working directly with individuals, is preferred but not required.

Able to promote cooperation between social services and other department employees.

Must be able to be professional over the phone and in person when representing the company.

Must have excellent communication skills and take pride in his/her performance.
