Ohioans Home Healthcare & Hospice is looking for a Part TimeChaplain to help patients and family members to navigate through the end-of-life process. As a Chaplain , you will be responsible for the provision of spiritual care services to patients and families/caregivers of Ohioans Home Healthcare and Hospice, either directly or through coordination of care with other spiritual counselors. Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
Facilitates the development of the individualized the plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
Provides direct spiritual care to patients and families/caregivers.
Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.

Bachelor’s degree or equivalent Theological degree and experience with families/caregivers and patients through death and/ dying issues-or- Master’s degree in Social Work with qualifications of experienced clinical grief counseling
Ability to network with community clergy and congregations

Our team provides post-operative care, offering continual monitoring of chest pain, blood pressure, edema, and education on signs and symptoms of further cardiac problems.
The team will inform patients about the disease process of chronic obstructive pulmonary disease, congestive heart failure, diabetes, renal failure, multiple sclerosis, osteoporosis, and more.
