To provide the highest quality of care to Accord Services clients utilizing the Plan of Care provided to ensure client’s needs are met in relation to safety, well-being and satisfaction.

Provide medical and personal care for the client based on individualized Plan of Care prepared by Nurse Supervisor available in the client’s home, resulting in satisfaction for all parties involved.

Orient new caregivers to cases and clients’ plan of care, ensuring clients’ clinical needs are met.

Provide prompt communication of changes in client medical status/emergencies to Nurse Supervisor, enabling effective management of cases.

Provide accurate and timely required documentation (i.e. declining to participate in all parts of Plan of Care) to ensure effective management of cases and DHR compliance.

LPN requirements:

High school diploma/GED

Certificate from accredited nursing program
