Job Description Licensed Practical Nurse (LPN) Central Nursing Home – Chicago, IL

Central Nursing Home is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our nursing team. Our LPNs play an important role in providing quality, resident-centered care while working collaboratively with CNAs, nursing leadership, physicians, and other members of the interdisciplinary team.
We currently have opportunities available across AM, PM, and NOC shifts .

Responsibilities
Provide safe and appropriate nursing care to residents
Administer medications and treatments in accordance with physician orders and facility policies
Monitor residents and promptly report changes in condition
Complete accurate and timely nursing documentation
Assist with the development and implementation of resident care plans
Supervise and appropriately delegate responsibilities to CNAs
Communicate effectively with residents, families, physicians, and members of the interdisciplinary team
Collaborate with the Director of Nursing, Assistant Director of Nursing, and other nursing leadership
Maintain resident dignity, privacy, safety, and quality of care
Follow facility policies, procedures, and applicable regulatory requirements
Perform additional nursing duties as assigned

Qualifications
Current and valid Licensed Practical Nurse (LPN) license in the State of Illinois
Graduate of an accredited practical nursing program
Strong clinical, communication, and organizational skills
Ability to supervise and appropriately delegate to nursing staff
Dependable, professional, and committed to resident-centered care
Must meet applicable health and background requirements

Schedule Available shifts include:

AM: 7:00 AM – 3:30 PM

PM: 3:30 PM – 12:00 AM

NOC: 12:00 AM – 8:30 AM

Rotating weekends and weekend availability may be required
