Physical Therapist ( PT ) - Home Care - Full Time

Role: Physical Therapist (PT)

Schedule: Full-time Monday-Friday + occasional weekends (as needed)

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Denver. Our mission is to dramatically improve patient access to home health care.

The PT role is a core part of that mission: you restore function, keep patients safe at home, and drive real outcomes.

We also build technology to support field clinicians. Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.

Complete comprehensive in-home assessments and deliver skilled PT visits.

Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.

Manage treatment and outcomes directly, including PTA supervision where appropriate.

Document promptly and thoroughly — defensible, efficient, and consistent.

Active Colorado PT license (in good standing)

Strong clinical judgment and comfort working independently in the field

Comfortable escalating changes in condition to RN/MD and documenting rationale clearly

Current driver’s license and comfort with daily travel across assigned territory

Able to carry/transport basic therapy equipment as needed

Competency in core home health PT scope

Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations

Patient/caregiver coaching with practical home programs

Prior home health experience

Axxess experience (or similar home health EMR)

Deeper clinical experience in high-signal PT populations

Experience supervising/mentoring PTAs and collaborating on care plans
