Travel Hospice LPN

American Traveler is hiring a Home Health LPN with 2+ years of home health and hospice experience for a day-shift assignment with no call requirements and a strong focus on work-life balance.

Job Details

• Home health and hospice setting providing in-home care to adult patients,
• Day shift schedule, Monday through Friday, approximately 8:00 AM to 4:30 PM or 8:30 AM to 5:00 PM,
• Weekends free with no call or after-hours work required,
• Typical patient visit load of 5-6 per day, with scheduling adjusted for drive time and distance,
• Driving distances vary daily, ranging from 5-10 miles up to 100 miles depending on location,
• Average visit length of 45-60 minutes allowing for focused one-on-one patient care,
• Epic EMR experience required,
• Patient ratio of approximately 1:30,
• Must be willing to float within the health system as operationally needed,

Job Requirements

• Active LPN license required if applicable,
• Minimum 2 years of home health experience required,
• Hospice experience required,
• Current BLS certification required,
• Epic charting system experience required,
• Skills in IV therapy/phlebotomy, central line care, colostomy/ileostomy management, and foley catheter insertion preferred,
• Two professional references from within the past 2 years required, with at least one from a supervisor,
• Proof of Washington state license application required for consideration,
• Must not have more than one DUI on record,

Additional Information

• Perform focused assessments within LPN scope of practice, incorporating a holistic approach to patient needs including psycho-social, nutritional, and home environment factors,
• Provide basic and complex wound care, ostomy care, and patient education on conditions such as heart failure, diabetes, COPD, and cardiopulmonary management,
• Support care-plan development and progression through clinical observations and input,
• Work within a supportive, close-knit team environment with strong mentorship for professional growth,
• First-time travelers are not accepted for this position,
• Travel experience required,
• Candidates must not be local, with local defined as within 60 miles of the facility,
• Previous PeaceHealth employees must have been separated from the health system for at least 1 year before returning as a traveler,
• Up to 7 RTO days may be approved prior to contract signing, with no additional time off approved afterward,
• Mileage reimbursement available for driving between patient visits
