Outdoor Facilitator

Under the direction of the Wraparound Leadership, the Wraparound Facilitator carries a caseload of children and/or adolescents and their families. The Wraparound Facilitator conducts monthly and as needed meetings with families to assess progress toward their goals and communicates progress or barriers to treatment to the referring party. The Wraparound Facilitator frequently communicates and consults with team members in developing and maintaining a Plan of Care and Crisis Safety plan for families. The Wraparound Facilitator is responsible for completing all required documentation in accordance with agency and County standards.

Helps develop family-centered help-giver skills
Helps to achieve professional and family partnerships
Assists family to incorporate informal supports, natural family helpers, and other community resources
Assists/completes outcome measures with child and families
Coordinate, and conduct monthly and as needed Child Family Team meetings with families to assess their needs and progress toward goals
Maintain client chart in accordance with HIPAA, DMH, DCFS, Probation and Los Angeles County standards
Suspected child abuse reports
Respond to crisis and coordinate immediate intervention to stabilize family
Leads the individual Child Family Team by:
Ensuring that all the strengths and needs are identified and documented
Being the contact point for children, families, service providers and the community
Assess the need to request flex funds and communicate need to immediate supervisor

Drive personal vehicle as necessary

Actively participate in the quality improvement project

The Wraparound Facilitator will take on special projects or additional duties as assigned by the Wraparound Leadership and/or AVP of ICBS
Ability to speak, read, write and understand English

• Must have strong organizational and planning skills
• Knowledge of social services system and community resources
• Knowledgeable in Wraparound philosophy, policy, phases and process
• Demonstrate skill in operating Microsoft Word, Outlook and Excel
• Skilled in relationship building, conflict mediation and facilitation a plus
• Bilingual (English/Spanish) preferred

Bachelor's degree in Human Services, Social Work, Psychology or related field or 3 years of experience working with high-risk children or youth in either Child Welfare, Probation or Mental Health systems

• Department of Justice/Fingerprint clearance
• Proof of legal eligibility to work in the United States
• While performing the duties of a Wraparound Facilitator, the employee is frequently required to walk, drive, and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to site, stand and reach. The work environment is indoors and outdoors. The work environment is community-based and requires conducting home visits.
