About the Role

Stern at Home Therapy provides outpatient-quality physical therapy in the home—giving clinicians the flexibility, autonomy, and support to deliver truly patient-centered care.

This is an ideal role for a Physical Therapist who values 1:1 treatment, schedule control, and a better work-life balance .

Responsibilities

Provide skilled physical therapy services in patients’ homes

Perform evaluations and develop individualized care plans

Deliver high-quality, 1:1 patient care (primarily geriatrics)

Complete timely documentation in EMR

Collaborate with a supportive clinical team as needed

Qualifications

Licensed Physical Therapist in New York

Strong clinical and interpersonal skills

Ability to work independently in a home-based setting

Home care experience preferred, but not required
