Registered Nurse

We are seeking a compassionate and dedicated Home Health Registered Nurse (RN) to provide high-quality, patient-centered care in the comfort of patients' homes. The Home Health RN is responsible for assessing patient needs, developing individualized care plans, coordinating services, and providing skilled nursing care to patients recovering from illness, injury, surgery, or managing chronic conditions.

The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for helping patients maintain independence while achieving their healthcare goals. This position requires collaboration with physicians, therapists, caregivers, and interdisciplinary team members to ensure optimal patient outcomes.

Current, unrestricted Registered Nurse (RN) license.

Graduate of an accredited nursing program.

Minimum one year of nursing experience required; home health experience preferred.

Knowledge of Medicare, home health regulations, and clinical documentation standards.

Valid driver's license, reliable transportation, and proof of auto insurance.

Conduct comprehensive patient assessments and develop individualized plans of care.

Provide skilled nursing services, including wound care, medication management, IV therapy, and disease management education.

Monitor patient progress and adjust care plans as needed.

Coordinate care with physicians, therapists, social workers, and other healthcare professionals.

Identify changes in patient condition and communicate appropriately with the healthcare team.

Ensure compliance with agency policies, state regulations, and Medicare guidelines.
