Home Infusion Nurse - Partner with Patients. As a Home Infusion Nurse, you'll provide one on one, skilled clinical care to patients where they feel most comfortable-at home or in an ambulatory infusion suite. This role offers true autonomy, a flexible schedule, and the opportunity to build meaningful connections with patients and caregivers while delivering life changing therapies.

You'll have the support of a multidisciplinary care team, comprehensive training, and the resources you need to deliver safe, effective, and compassionate infusion care.

• *Travel within the Warwick, RI area to deliver care in patient homes or designated sites
• *Travel to patient homes and related sites using a personal or rental vehicle in accordance with company travel policies

• Provide safe and compliant infusion services in accordance with Optum Infusion Pharmacy policies, protocols, and physician orders
• Establish and maintain positive therapeutic relationships with patients and caregivers, including education on self-care and self-administration
• Assess patient suitability for home infusion therapy, including home safety, caregiver support, and patient independence
• Obtain and maintain accurate patient medical histories, clinical assessments, and all required documentation, including plans of care, visit notes, medication profiles, and discharge summaries.
• Deliver skilled nursing care across a range of patient ages and conditions based on physician orders and treatment plans
• Administer medications and monitor patient responses, including managing infusion reactions, vital signs, side effects, and lab collection
• Communicate patient status and therapy response promptly with physicians, pharmacists, supervisors, and other care team members
• Assist in staff orientation and training, serving as a clinical educator and resource as requested

**Active and unrestricted Registered Nurse (RN) license in the state of Rhode Island

• 1+ years of RN experience in a clinical setting supporting infusion or IV therapy (e.g., ICU, Med Surg, ED, PACU)
• Familiarity with Microsoft Office and EMR platforms
• Knowledge of Infusion Nursing Society (INS) standards
• Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29.00 to $52.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
