Senior Community Care Advanced Practice Clinician

$40,000 Student Loan Repayment Or $30,000 Sign-on Bonus

This role requires travel across the entire state of PA, DE, NC, SC and GA, approximately 85% of the time. All travel expenses are covered, and there is likewise a base salary plus multiple incentives. As a team member of our Senior Community Care (SCC) team, we work to provide care to patients in nursing homes, senior housing and assisted living settings. This life-changing work adds a layer of support to improve access to care. We're connecting care to create a seamless health journey for patients across settings. The Senior Community Care (SCC) program is an integrated care delivery program that coordinates the delivery and provision of clinical care of members. The Advanced Practice Clinician within SCC provides care to our highest-risk health plan members. We're fast becoming the nation's largest employer of Nurse Practitioners; This growth is not only a testament to our model's success but the efforts, care, and commitment of our Providers.

Past medical history review, medication reconciliation, vital signs, and physical exam

Evidence-based screenings and point-of-care testing (as appropriate)

Support members during a short term sub-acute rehabilitation episode

Communicate and collaborate with the interdisciplinary care team

Conduct advanced illness and advanced care planning conversations

For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

1+ years of clinical experience as an APC (long-term care setting preferred)

• Experience in geriatric medicine, long-term care, senior living or home care setting
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Proficiency with electronic medical records and technology

Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
