Compensation Structure

Salary range: $80,000 - $110,000/year

Start of Care (SOC): 2.0 Units

Evaluation / Re-Evaluation /Recertification / Resumption of Care: 1.5 Units

Routine Visit: 1.0 Unit

OASIS Discharge: $65

Responsibilities

Conduct comprehensive assessments of patients’ health needs and develop individualized care plans.

Administer medications and treatments as prescribed, monitoring for effectiveness and side effects.

Provide education and support to patients and their families regarding health management, disease prevention, and treatment options.

Perform wound care, catheter care, and other nursing procedures as needed.

Document patient progress accurately in electronic health records (Epic) and maintain compliance with regulatory standards.

Collaborate with physicians, therapists, and other healthcare professionals to coordinate care effectively.

Ensure infection control protocols are followed to maintain a safe environment for patients.

Participate in ongoing professional development and training opportunities.

Qualifications

Active Florida RN License

CPR Certification

Strong assessment skills

Home health/OASIS experience preferred but not required
