Home Care RN - Flex - Home Health

Deliver compassionate, patient-focused care as a Home Health Registered Nurse (RN) providing in-home services. This vital contract position specializes in home health, prioritizing high-quality care, clear patient communication, and collaboration with interdisciplinary teams.
Schedule Monday through Friday, 8:00 AM 5:00 PM
Key Responsibilities Perform comprehensive OASIS assessments and develop individualized care plans.
Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
Maintain thorough, accurate documentation using Home Care Home Base (HCHB).
Collaborate with therapists, social workers, and home health aides for coordinated care.
Uphold all regulatory and agency standards in documentation and direct care.
Qualifications and Preferred Background Active RN license in good standing within the state
At least 1 year of recent home health experience (field experience highly valued)
Proficient in Home Care Home Base (HCHB) and skilled with OASIS
Reliable transportation, valid drivers license, and auto insurance
Ability to thrive independently while managing a field-based, flexible schedule
Working knowledge of Medicare and compliance standards for home health
