Rehabilitation - Occupational Health - Occupational Therapist

Role Overview: The Home Health Occupational Therapist assesses and develops individualized therapy programs to improve patients' strength, mobility, and independence in their homes. They collaborate with physicians and healthcare teams to create and update treatment plans, provide therapeutic interventions, and instruct patients and families on rehabilitation techniques. The therapist reviews and expands retraining programs, designs community reintegration activities, and recommends adaptive equipment to enhance daily living and work environments. They supervise and evaluate therapy staff, document patient progress accurately, and participate in care coordination and discharge planning.

Qualifications & Skills: Candidates must have a degree from an accredited Occupational Therapy Program, a valid OT license, and current CPR certification. Preferred candidates have at least six months of experience, with home health experience considered a plus. Strong organizational and communication skills are essential, along with a valid driver’s license, auto insurance, and reliable transportation.

Key Duties Assess and develop therapeutic plans with measurable objectives.

Coordinate with healthcare providers and participate in treatment plan revisions.

Deliver therapy services to improve physical functions and teach compensatory strategies.

Supervise therapy assistants and aides, and ensure proper documentation of patient care.

Assist in care coordination, discharge planning, and community reintegration activities.

Design and train on adaptive equipment and environmental modifications.
