LPN - Union Position Full Time

Licensed Practical Nurse - Lpn (Union)
Seeking dedicated Licensed Practical Nurses for Full Time or Part Time shifts at our 31-bed intermediate care facility in Commack.
Schedule: 7:00am-3:30pm, 3:00pm-11:30pm, or 11:00pm-7:30am. All F/T and P/T LPN shifts at this location require at least one weekend shift per week on a recurring basis.
General Purpose of the Job: Under the supervision of a Registered Nurse, assist in the overall health and safety of the individuals served.
Essential Duties and Responsibilities include the following. Other duties may be assigned.

• Knowledge of all medications for all individuals.
• Administer medications and treatments as prescribed by physician.
• Generate incident, seizure, and illness reports.
• Monitor and report health care on daily flow report records.
• Instruct mid-level supervisors and direct care staff in above, i.e., hygiene, diet, weights, etc.
• Submit written monthly, quarterly, annual nursing/medical progress notes.
• Coordinate laboratory care and medical appointments (routine, fees, special and emergency).
• Maintain functioning interpersonal relationships with other disciplines.
• Maintain ongoing communication with other programs, i.e., client health care.
• Monitor program participants during mealtimes.
• Order, record, and maintain sufficient/adequate supply of medication.
• Maintain sufficient/adequate amount of medical supplies/materials.
• Maintain accurate medical records including medication administration sheets, Kardex, Formulary, etc.
• Provide emergency first aid. Coordinate emergency services.
• Supervise direct care staff in absence of mid-level supervisor.

Education and/or Experience: Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience.
Certificates, Licenses, Registrations: New York State Licensed Practical Nurse
