Role Overview:

Provides direct patient care in a Wound Care Center, focusing on assessment, diagnosis, planning, implementation, and evaluation within nursing standards.

Responsibilities Include

Promoting patient and family-centered care, educating on illness management, post-treatment care, and medication administration.

Participating in activities that ensure patient safety, quality, and regulatory compliance.

Developing and documenting individualized care plans collaboratively with patients, families, and the healthcare team.

Assessing physical, psychosocial, and emotional needs, considering patient preferences, values, and cultural factors.

Ensuring communication of care plans across the continuum and proactively managing patient needs.

Qualifications & Skills

Minimum education: Nursing diploma or associate's degree; RN licensure required.

Preferred: Bachelor's degree in nursing and less than 2 years of experience.

Strong assessment, critical thinking, communication, and teamwork skills.

Ability to work safely and maintain alertness, with a focus on patient safety.
