Licensed Practical Nurse

St. Joseph's Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our nursing team in a community-based Intermediate Care Facility home. This home supports six residents, providing a safe and nurturing environment where dignity and independence are prioritized.

Provide direct nursing care and health monitoring for residents.

Administer medications accurately and monitor for side effects.

Develop, implement, and evaluate individualized care plans.

Maintain medical records and ensure compliance with ICF/ID regulations.

Assist residents with activities of daily living (hygiene, dressing, feeding).

Accompany residents to medical and dental appointments.

Respond to health emergencies and implement appropriate interventions.

Collaborate with the interdisciplinary team and participate in care planning.

Maintain adequate medical supplies and equipment.

Support residents in community activities and outings.

Competitive Pay - Starting at $27.50/hour + $2.00/hour shift differential, $5,000 sign-on bonus

Current LPN license in Pennsylvania

CPR certification

Valid PA driver's license

Experience with individuals with intellectual and developmental disabilities preferred

Ability to lift/move 2550 lbs and assist with transfers

Strong communication and teamwork skills
