Licensed Practical Nurse - Primary Care Clinic - Full Time

The Registered Nurse (RN) provides integrated, person-centered nursing care within a Certified Community Behavioral Health Clinic (CCBHC). This role supports individuals with mental health and substance use disorders through assessment, coordination of care, medication support, and collaboration with interdisciplinary teams. As part of the CCBHC model, the RN plays a key role in delivering whole-person care, including behavioral health, physical health monitoring, and crisis response services.

Essential Functions Conduct comprehensive nursing assessments, including physical and behavioral health screenings

Monitor and document vital signs, medication adherence, and overall health status

Collaborate with providers (psychiatrists, PMHNPs, therapists, care coordinators) to support integrated treatment planning

Support care coordination with hospitals, primary care providers, and community resources

Assist with intake, risk assessments, and ongoing clinical documentation

Ensure compliance with CCBHC standards, state regulations, and organizational policies

Document all services provided within expected time frames, meet service definitions and regulatory standards, person-centered and meet quality improvement goals

Participate in and complete all required training courses accurately and timely

Participate in on-call nursing support

Complete other administrative support duties and special projects as developed and assigned

CCBHC-Specific Expectations Deliver services aligned with a whole-person, trauma-informed care model

Participate in care coordination across multiple systems (medical, behavioral, social services)

Provide care regardless of a client’s ability to pay or insurance status

Qualifications and Education Active Registered Nurse (LPN/RN) license in the State of New Mexico

Experience in behavioral health, substance use treatment, or community health preferred

Knowledge of psychiatric medications and co-occurring disorders

Preferred Experience Experience working in a CCBHC or integrated care setting

Bilingual (English/Spanish) preferred

Knowledge and Skills Demonstrated success in psychological therapies, building a therapeutic alliance, dealing with challenging behavior and the administration of psychiatric medication
