Our Hospice Nurse Care Manager (RN) Will: Provide individualized care by treating each patient as they would want a loved one treated.
Develop care plans that reflect each person and their needs.
Be detail-oriented, knowing that carefully managed patients (medications, treatments, communication, aide supervision, etc.) have more comfort and dignity.
Focus on customer service when interacting with patients, families, communities/facilities, and all partners and co-workers.
Help team members improve through instruction and support.
Build and manage meaningful relationships with all partners.
Our Hospice Nurse Care Manager (RN) Has: An active state-required RN license or a compact state license.
2+ years of hospice experience.
An active Driver's License with a reliable vehicle, insurance, and an acceptable driving record.
The ability to meet the physical requirements for patient care with reasonable accommodations (e.g., Independently lift at least 30lbs).
