Oncology Nurse Navigator

The Oncology Nurse Navigator provides expert individualized care coordination to patients, and their families ensuring a timely schedule for diagnostic work-up, results reporting and communication between the patient and the multidisciplinary team ensuring the patient receives necessary referrals and follow-up. The Oncology Nurse Navigator serves as an advocate for the patient, promoting timely, competent, compassionate care and enhances the physician/patient relationship improving the experience of the patient with cancer.

Collaborates and continuously partners with chronically ill or high risk patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach.

Prepares patients and caregivers for what to expect throughout the cancer care continuum.

Creates and promotes adherence to a care plan, developed in coordination with the patient, PCP, oncologist and family/caregiver(s).

Reduce emergency room utilization and hospital readmissions through outpatient symptom management.

Follow patients throughout the care continuum including inpatient admissions in collaboration with inpatient care management resources.

Support transitions of care and coordination between SKCC sites.

Educate multidisciplinary colleagues about the role of the Nurse and Patient Navigators, identified barriers, available resources and the navigation process.

Graduate of an accredited school of nursing required; Three (3) years of Healthcare/Medical, experience in Oncology/Cancer Care preferred.
