Certified Nursing Assistant (CNA)
We are seeking a compassionate, dependable, and dedicated Certified Nursing Assistant (CNA) to provide exceptional care and support to individuals in residential and community-based settings. In this rewarding role, you will help individuals achieve their highest level of independence, health, and quality of life while working as part of a collaborative team.
The CNA is responsible for ensuring the health, safety, supervision, and well-being of individuals served in accordance with their individualized care plans. This position provides direct personal care, medication administration, healthcare support, community integration, and assistance with daily living activities. Direct Care & Support

Provide compassionate, person-centered care and supervision to individuals receiving services.
Oral hygiene
Administer medications and prescribed treatments in accordance with physician orders, organizational policies, and regulatory requirements.
Accompany individuals to medical appointments, recreational activities, community events, and other outings.
Provide support in a variety of settings, including residential homes, community locations, medical facilities, and hospitals.
Assist in monitoring individual progress, skill development, and environmental factors that affect well-being.
Support the implementation of Individual Support Plans (ISPs) and care plans.
Maintain required staffing ratios and supervision levels as outlined in ISP requirements.
Accurately document services provided, progress toward goals, and significant events.
Participate in team meetings, care planning discussions, and program development activities.
Health, Safety & Environment
Monitor for and promptly report safety concerns, maintenance issues, health changes, or incidents.
Collaborate with the RN and Program Director to ensure effective healthcare coordination and regulatory compliance.
Promote the health, dignity, rights, and independence of individuals served.
Administrative Responsibilities
Perform administrative tasks and other duties as assigned.
Current Certified Nursing Assistant (CNA) certification in good standing with the state nursing board.
High School Diploma or GED.
Client Care & Support
Strong ability to provide quality personal care and assistance with daily living activities.
Ability to maintain a safe environment and respond effectively to medical, behavioral, or emergency situations.
Knowledge of safety protocols and infection-control practices.
Communication & Relationship Building
Ability to build positive relationships with individuals served, families, guardians, and team members.
Ability to work effectively with nurses, supervisors, healthcare providers, and interdisciplinary teams.

Ability to maintain confidential and compliant records.
Comfortable working in multiple service environments, including homes, communities, and healthcare settings.

Ability to effectively prioritize responsibilities and manage multiple tasks throughout the workday.

Ability to meet the physical demands of direct care, including lifting, transferring, standing, bending, and active support.

This position requires regular physical activity and the ability to safely assist individuals with mobility and daily living tasks.
Lift, move, and transfer individuals weighing up to 50 pounds or more, with or without assistance.
Climb stairs and navigate various community environments.
Drive agency or personal vehicles as required.
Sit for meetings, training sessions, and documentation tasks.
Residential homes, community settings, healthcare facilities, hospitals, and office locations.
Well-lit indoor environments with occasional exposure to varying community and environmental conditions.
May require working evenings, weekends, holidays, or flexible schedules based on program needs.
Follow all agency policies regarding Fire Safety, HIPAA, Human Resources, and confidentiality.
Demonstrate knowledge of Bloodborne Pathogens, Right-to-Know regulations, Security Practices, and Infection Control procedures.
