Required Qualifications:
Physical Therapist, Outpatient
PT (Compact Accepted)
Certifications: BLS (Copy needed)
SSN Required
DOB Required
References: 1 reference in entire work history
License, BLS, and Outpatient experience are required.

Preferred Qualifications: Travel experience preferred.
