Perform physical examinations, obtain medical histories, diagnose illnesses, develop treatment plans, and prescribe medications and therapies in accordance with standardized procedures and applicable scope of practice.Provide preventive and primary care services, including immunizations, health screenings, and wellness counseling.Manage chronic conditions such as diabetes, hypertension, asthma, and depression, with a strong focus on patient education and self-management support.Document all clinical encounters thoroughly and accurately in the Electronic Health Record (EHR) system.Collaborate with an interdisciplinary care team to address medical, behavioral, and social determinants of health.Provide acute care visits and triage support as needed.Support transitions of care, referrals, and follow-up for diagnostic tests and specialty care.Participate in clinical huddles, staff meetings, and quality improvement initiatives.Maintain awareness of and adherence to clinical protocols, policies, and regulatory requirements.Serve as a preceptor for students or trainees when appropriate.Demonstrated commitment to providing care to underserved communities and vulnerable populations.Master’s or Doctorate degree from an accredited Nurse Practitioner program.Current California Registered Nurse (RN) license and Nurse Practitioner license.National certification as an NP in Family, Adult-Gerontology, or a relevant specialty.Minimum 1–2 years of clinical experience preferred (new grads may be considered).Experience in a Federally Qualified Health Center (FQHC) or community clinic setting.Proficiency in Epic EHR system.Bilingual or multilingual skills.
