Licensed Practical Nurse (LPN)
Seeking a Licensed Practical Nurse (LPN) for a post-acute care unit to provide skilled nursing services to long-term and rehab patients. The role involves patient care, documentation, and occasional floating to other units as needed.
Administer medication and treatments using EPIC system documentation.
Monitor and assess patient conditions, including IV therapy and respiratory support.
Document care activities accurately and timely, ensuring compliance with facility protocols.
Collaborate with the healthcare team to develop and implement patient care plans.
Float to different units as required, supporting staffing needs.
Ensure compliance with all safety standards and infection control procedures.
Valid LPN license with active file on record.
Minimum of 1 year of LPN experience, preferably in LTC/SNF settings.
Experience with EPIC electronic medical records within the past 2 years.
Prior travel experience required.
Dementia-capable care certification preferred.
