Full-time RN / LPN Home Health Care

Position Status: Full Time - Day Shift | Location: Sentara Home Care Services - Chesapeake, VA
Pay Range: $22.39 - $37.32 per hour. Mileage reimbursement is provided for miles driven between patient visits.
Responsibilities Provide direct, one-on-one care to patients in their homes.
Monitor patient health, administer treatments, manage medications, and offer essential support for various medical needs.
Collaborate with Registered Nurses, physicians, and other healthcare professionals to deliver high‑quality, compassionate care.
Serve as a patient advocate and participate in prevention of illness or disease.
Carry out nursing interventions, care plans, customer service, testing, and medication administration.
Support a holistic approach to patient care, including clinical support and teaching for patients and family members.
Education & Certification Trade School Graduate (Required)
LPN License (Active in Virginia) - Required
Basic Life Support (BLS) - Required within 90 days of hire
Experience Minimum 2 years of clinical experience (required)
Infusion experience with ports, central lines, and peripheral IVs (required)
