URGENT LPN/RN POSITIONS NEEDED FOR HOME HEALTH CARE

Homecare with Heart is offering you the most competitive pay scale in the area, with pay for LPN at $26‑28 per hour depending on experience and availability. The role involves providing skilled visits in patients’ homes. This position will be serving Trumbull County.

Responsibilities

Administer medication in accordance with standards of practice.

Encourage social habilitative skills and encourage self‑care and independence.

Perform client treatments in accordance with Plan of Care.

Assist the consumer in learning appropriate self‑care techniques.

Prepare clinical and progress notes to document care provided.

Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor.

Promote activities to achieve a safe and healthy environment.

Observe aseptic technique when performing client procedures/care.

Collaborate with other members of the clinical team to develop and improve the Plan of Care.

Assist the client in achieving established goals in the Plan of Care developed by the RN.

Maintain productivity according to the standards of the agency.

Maintain license, CEUs and mandatory employment requirements.

Participate in client care meetings, in‑services, orientation and staff meetings.

Provide service in accordance with agency’s philosophy, standards, policies and scope of practice; participate in Performance Improvement and adhere to HIPAA guidelines.

Other duties as assigned.

Qualifications

Valid State LPN License.

Valid driver’s license; use of an automobile is required due to travel involved.

Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities.

Ability to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.

Ability to use a computer, laptop or tablet.

Ability to think independently, recognize potential problems, gather information, and know when to contact a supervisor.

Commitment to excellent customer service and exceptional client care while staying within the rules of the agency and rules of each program served.
