Meaningful, one-on-one patient care focused on comfort and quality of life

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

You'll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Lead patient-centered hospice care

Develop and manage individualized, comfort-focused care plans

Deliver skilled nursing care with a focus on pain and symptom management

Serve as the care coordinator

Act as case manager, coordinating care across the interdisciplinary team

Collaborate with physicians, social workers, chaplains, and aides

Monitor patient condition and adjust care plans to maintain comfort

Help families navigate complex decisions with empathy and clarity

Build meaningful, trusting relationships in deeply personal moments

Drive quality & compliance

Participate in IDT meetings and care planning

Support quality and compliance standards

Active RN license (or compact eligibility)

Graduate of an accredited nursing program

Minimum 1 year of clinical nursing experience

Strong clinical judgment and compassionate care approach

Experience in hospice, palliative care, or home health

Case management and interdisciplinary care experience

Experience with Homecare Homebase (HCHB)

Field-based care in homes, facilities, and community settings
