Role Registered Nurse providing weekend home health visits for adult and geriatric patients.
Responsibilities Conduct in-home assessments, develop and update care plans, administer medications and treatments, provide wound care and IV therapy, and coordinate with physicians, pharmacists, and interdisciplinary teams to ensure quality care.
Qualifications & Skills Compassionate and patient-focused, with strong documentation skills in EHR systems. Ability to educate patients and families on disease management, medications, and home safety. Knowledge of infection control standards.
