Are you a Licensed Practical Nurse looking for a new opportunity? Haven Hospice is seeking a passionate, dedicated Hospice LPN to join our team in Lake City, FL . Our hospice LPNs provide expert, patient-centered care at a critical time in people’s lives. Provide 1:1 care to make a lasting impact on patients and families
~ Greater work/life balance with flexible scheduling options
~ As a Hospice LPN You will:
- Give hands-on care to patients nearing the end of life, following state nursing rules.
- Follow the care plan after assessing the patient’s needs.
- Keep clear and timely records of the care provided.
- Teach and support patients and their families about the illness and care plan.
- Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers).
Take part in training and quality improvement activities to grow professionally and improve care.
