Job Summary Join our compassionate and collaborative team as a Community Behavioral Health Liaison, where you will play a vital role in connecting individuals to critical behavioral health and community-based services. In this role, you will act as a liaison between clients, providers, and community resources to ensure coordinated, person-centered care. You will help identify client needs, facilitate service connections, and support collaboration across multidisciplinary teams to improve outcomes. Key Responsibilities Serve as a liaison between clients, providers, and community resources to ensure coordinated care

Engage individuals and assess needs related to behavioral health, social services, and community support

Facilitate referrals and connect clients to appropriate services including medical, mental health, housing, and social supports

Build and maintain relationships with community partners and service providers

Maintain accurate and timely documentation in accordance with organizational and regulatory standards

Support outreach efforts and community engagement initiatives

Education, Experience, and/or Credential Qualifications Bachelor’s degree in social work, psychology, counseling, or a related human services field preferred

Relevant experience in behavioral health, case management, or community services may be considered in lieu of education

Knowledge of community resources and behavioral health systems preferred

Additional Qualifications Must have a valid driver’s license with an acceptable driving record and current auto insurance

Strong communication and relationship‑building skills

Ability to collaborate with interdisciplinary teams and community partners

Ability to perform repetitive hand, finger, and arm movements for typing and documentation
