Dietitian care / nursing home

Conduct comprehensive Nutrition assessments to determine the nutrition needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.

Deliver and document direct care Nutrition interventions in accordance with physician orders and/or as agreed upon in the participants' care plans including but not limited to implementing and monitoring special diets and meal intake and providing nutrition counseling to participants and families.

Oversee the menu planning and meal delivery program to ensure that all safety and regulatory requirements are met, and participants receive the correct special diets.

Provide staff training to nursing and dietary staff on the meal delivery program as needed.

Manage the Nutrition staff including staff including hiring, training, supervising and mentoring.

Follow state, federal and facility policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.

Bachelor's Degree preferred.

• Registered Dietician (RD) Credential by the Commission on Dietetic Registration (CDR).
• Minimum of four years' experience managing nutrition services in a similar setting.
• Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.
• Ability to work independently with minimal supervision.
