Bilingual Care Advocate (Remote)

Maternity Care Coalition (MCC) is a community-based nonprofit organization serving Pennsylvania and Delaware with over four decades of commitment to improving maternal and child health and early care and education. MCC’s mission is to improve the health and well-being of pregnant women and parenting families and enhance school readiness for children ages 0-3. We achieve this through direct service, advocacy, and research, in collaboration with individuals, families, providers, and communities. Responsible for providing culturally sensitive group education and case management to pregnant and newly parenting women, including assessment, referral, follow-up, monitoring, and needed supportive services. Provide home visiting services for families. Services are provided during regular business hours, in the evenings, and on weekends, as needed.

Participates in outreach efforts to locate and communicate with pregnant women and new parents and direct them to needed health and social services.
Provides intensive case management, including assessment, referral, follow-up, and monitoring to high-risk pregnant women, new parents, and their families, through routine home and office visits and regular phone contact.
Supports clients in planning for their own health and social service needs by assisting clients in identifying goals.
Learns about and contacts community resources clients need.
Conducts health promotion activities by providing client education, disseminating educational literature, and distributing disease prevention materials, including condoms.
Ensures timely scheduling of prenatal, postpartum, well-child, and early intervention appointments.
Facilitates (or co-facilitates) community health education programs
Attends community meetings, health fairs, and represents MCC as requested by supervisor.
Routinely uses MCC client database and other computer systems including e- mail, Internet, and word processing.
Attends orientation and ongoing training sessions on maternal and child health and welfare, child abuse, housing, nutrition, substance abuse, counseling, parenting, infant and family support issues.
Participate in professional development, training, and workshops to enhance professional growth.
Complete special projects and other duties as assigned to support departmental priorities.

Bilingual language skills, ability to speak, read, and write in both Spanish and English.
Knowledge of the concepts and practices associated with community health issues, particularly with pregnant women, prenatal care, parenting, and other services for mothers, young children, and their families.
Ability to travel in and around the Greater Philadelphia region to conduct home visits and community outreach.
Knowledge of and sensitivity to issues facing the community served.
Experience in client management procedures for determining eligibility, assessing needs, identifying resources, making referrals, following up, and documenting client interactions.
Strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, community partners, clients, and members of the community.
Ability to operate a computer and use a variety of common software programs, including Microsoft Office, spreadsheets, and customized databases.
Effective and accurate documentation of case notes and client data according to established client management documentation procedures.
Two-plus years of experience providing direct services in a community outreach setting
Experience with community health issues
Experience working with maternal and child health issues

High school diploma or equivalent required.
Bachelor's degree in Social Work, Public Health, or a related field preferred.

The assigned work arrangement for this position may include a combination of remote and onsite work and will be determined based on operational needs, role responsibilities, and performance. Staff working remotely are responsible for maintaining a safe and appropriate workspace.
Ability to engage in extended periods of sitting, computer work, and sustained concentration to complete financial analysis, reporting, and related tasks.
Ability to occasionally bend, reach, lift, carry, push, or pull training materials or equipment, generally not exceeding 30 pounds.
Ability to travel occasionally to attend conferences, seminars, and professional meetings.
