Flagstar Rehab COTAs work with a diverse and multi-disciplinary team to perform treatments and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Hourly Pay Rate
$33-$34 Hourly (W-2) , $37-$38 Hourly (1099)
Occupational Therapy Assistant Job Duties
Collaborate daily with OT Supervisor and follow established plan of care for all patients.
Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
Monitor and document patient progress with the use of innovative EMR software.

Occupational Therapy Assistant Requirements
Current NYS OTA license and registration.
EMR competence.
State-required medical documentation for work in a skilled nursing facility (physical exam, TB test, MMRV vaccine record or titers, COVID vaccine documentation).
Valid proof of identity and work authorization.
Malpractice/l liability insurance (if applicable).
