Travel nurse - home health home care

Registered Nurse – Home Health Case Manager in Bala Cynwyd, PA. This travel, temporary assignment starts July 22, 2026 for 12 weeks in a home health care setting. Weekly pay ranges from $2,329 to $2,412 per week, with guaranteed 40 hours per week. Shift: 8:00 AM–4:30 PM. Program: Local Contract (Nursing) within a home health care team. This role focuses on in-home patient management, care coordination, and clinical leadership across a high-volume caseload.

• Contract duration: Shift: 8:00 AM–4:30 PM (in-home visits)
• Home health care (travel temporary assignment)

Manage a caseload of in-home patients, conducting assessments and developing individualized care plans in collaboration with the interdisciplinary team.

• Perform OASIS data collection and documentation, ensuring accuracy and timely submission.
• Provide comprehensive case management, coordinating services, referrals, and equipment as needed to support patient goals.
• Monitor patient status, adjust plans as necessary, and communicate changes to the care team and supervising clinician.
• Maintain high-quality documentation and maintain productivity targets, typically serving a high-volume caseload with thorough documentation.

Active RN license in Pennsylvania (PA)

• 2+ years of home health experience
• Case management experience and OASIS experience required
• Wound care experience preferred
• Ability to perform in-home visits and operate within a home health care setting
