Job DescriptionJob DescriptionAbout the Role: D Square Healthcare Services Inc is looking for a compassionate and dedicated Direct Support Professional to join our team in Shakopee, MN. In this rewarding role, you'll make a meaningful difference in the lives of individuals with disabilities by providing personalized care and support. Responsibilities:Provide direct care and daily living support to individuals with intellectual or developmental disabilitiesAssist clients with personal hygiene, grooming, meal preparation, and household tasksImplement individualized support plans (ISPs) and document progress accuratelyTransport and accompany clients to medical appointments, community activities, and errandsAdminister medications and monitor health conditions per care plan guidelinesFoster independence, dignity, and community inclusion for each individual servedCommunicate effectively with supervisors, families, and healthcare team membersRequirements:High school diploma or GED equivalent requiredPrevious experience as a DSP, caregiver, CNA, or in a related healthcare/human services role preferredValid driver's license and reliable transportationAbility to pass a background check as required by Minnesota DHSCPR/First Aid certification (or willingness to obtain upon hire)Strong communication, patience, and problem-solving skillsGenuine compassion and commitment to person-centered careAbout Us:D Square Healthcare Services Inc is a trusted provider of direct support and healthcare services in the Shakopee, MN area, dedicated to enhancing the quality of life for individuals with disabilities.
