HopeWest is seeking a compassionate and collaborative Registered Dietitian (RD) to join our PACE team! In this role, you will have the opportunity to build meaningful relationships with participants while helping them achieve and maintain their health, independence, and quality of life.
As an integral member of our interdisciplinary team, the Registered Dietitian will assess nutritional needs, develop individualized nutrition plans, provide education and counseling, and collaborate with providers, nurses, therapists, and other care team members to deliver exceptional, participant-centered care.
PACE (Program of All-Inclusive Care for the Elderly) is designed to help older adults live safely and independently in their communities for as long as possible. Rather than treating one need at a time, our interdisciplinary team works together to provide comprehensive, coordinated care that supports the whole person.
At HopeWest, that means building relationships with participants, understanding their individual goals, and creating care plans that support not only their health-but also their independence, dignity, and quality of life.
As a Registered Dietitian, you will:
Develop, implement, and monitor individualized nutrition care plans.
Provide nutrition counseling and education to participants, families, and caregivers.
Collaborate with physicians, nurses, therapists, social workers, and other interdisciplinary team members.
Support participants with a variety of nutritional needs, including chronic disease management, weight changes, malnutrition risk, therapeutic diets, and other age-related health concerns.
Participate in interdisciplinary care planning and team meetings.
Help ensure meals and nutrition services align with participant needs, preferences, and prescribed dietary requirements.
Current registration with the Commission on Dietetics Registration (CDR).
Master's degree in dietetics, nutrition, or related field from an ACEND accredited program OR a Bachelor's degree if received before 2024.
ACEND-accredited supervised practice hours completed and passed the National Registration Examination for Dietitians required.
Experience working with older adults, individuals with chronic health conditions, or medically complex populations preferred.
Strong assessment, counseling, communication, and care-planning skills.
A passion for providing compassionate, person-centered care.
Competitive pay structure starting at $32.54-$36.00 per hour, depending on experience.
