The purpose of this position is to direct, supervise, evaluate and provide speech therapy services to patients in the home as prescribed by the attending physician.
We are looking for someone with at least 1 full year of supervised experience providing speech therapy services to patients in a clinical setting (No new grads), with some home health experience (preferred). At least a master's degree in speech language pathology from an ASHA accredited program; Active Speech Language Pathology license for State of Florida through board of SLPA; Current BLS Provider (BLS) CPR certification completed through the American Heart Association or American Red Cross (no online programs).
Evaluates patient’s speech and language abilities, both defects and assets, and performs periodic reevaluations.
Plans and provides rehabilitative services for speech and language disorders.
Records type of treatment and patient’s reaction to it on clinical/progress notes, which are maintained in the electronic medical record system.
Utilizes community resources and Agency personnel by proper referral.
Selects and administers diagnostic and therapeutic techniques and materials.
Instructs and counsels other health team personnel and family members in methods of assisting patient in improving, correcting, and accepting his disabilities.
Attends case conferences via phone or other methods as required.
Participates in the development and periodic revision of the physician’s plan of treatment.
Complies with federal, state, local legal and certification requirements by studying existing and new legislation, anticipating future legislation; Adheres to accreditation standards as set forth by the Accreditation Commission for Healthcare (ACHC).
*The employee/contractor will be expected to perform other reasonable related duties as assigned by management. The position will be as needed (per diem) with a rate as industry appropriate. Work will be performed in the patient home setting, with the therapist travelling to the patient’s home location for sessions.
