As a Hospice RN, you'll act as a trusted Case Manager, delivering care that supports patients in their homes and communities. You will guide families through care decisions, while providing comfort, dignity, and compassionate support during life's most tender moments.Our Commitment to YouJoin a mission-driven organization dedicated to compassionate end-of-life careCollaborate with a caring, interdisciplinary team that puts patients and families firstBuild meaningful relationships and make a lasting impact in the lives of othersWork in a culture grounded in respect, dignity, empathy, and serviceYour ImpactComplete comprehensive assessments of patients and families to determine hospice needsDevelop and implement individualized care plans, revising as neededProvide skilled nursing care, administer medications, and manage symptomsEducate patients and families on care techniques, medications, and comfort measuresMaintain accurate clinical documentation and communicate changes to physiciansParticipate in interdisciplinary team meetings and quality improvement initiativesProvide emotional support and counseling to patients and familiesSupervise hospice aides and delegate responsibilities appropriatelyCollaborate with an interdisciplinary team that shares your commitment to excellenceRequirementsCurrent RN license in the State of NevadaMinimum of 1 year of nursing experience, hospice or home health experience preferredHomecare Homebase (HCHB) and/or PointCare/PCC experience preferred, not requiredValid driver's license and reliable, insured vehicleStrong clinical skills and a passion for patient-centered careAbility to work independently and a passion for making a real difference
