Compassionate hospice and palliative care NP opportunity in Temecula, California. We are seeking an experienced Nurse Practitioner for a locum tenens role serving patients throughout central and north San Diego County in home-based settings.
Home-based hospice and palliative care
Start Date: ASAP
Schedule: Monday-Friday, 8:00 AM-5:00 PM
Shifts per Week: 5
Shift Duration: 8-hour days
Weekend Rotation: Rotating Sundays as needed, with a compensatory day during the week
Minimum 1 year of nursing experience in hospice care or palliative care
Active California Registered Nurse license
Active California Nurse Practitioner license
Completion of an accredited Nurse Practitioner program and MSN or equivalent advanced nursing education
Advanced Certification as a Hospice and Palliative Care Nurse (ACHPN) preferred; Strong documentation and computer skills
Bilingual Spanish skills preferred
Conduct comprehensive hospice and palliative care assessments in home settings
Evaluate patient history, perform physical assessments, and develop individualized plans of care
Collaborate with the interdisciplinary team, community physicians, patients, and families to coordinate care
See new admissions to the hospice program and manage a small patient panel
Facilitate goals-of-care discussions, advance care planning, advance directives, and POLST documentation
Document patient encounters and care plans accurately within the electronic medical record
Participate in interdisciplinary meetings, case conferences, and quality improvement initiatives
Serve as a clinical resource and educator for staff on hospice and palliative care best practices
