Care Management Pharmacist

Pharmacist PositionWe Are Looking ForA pharmacist who will play a personal role within our community to ensure that our patients receive the medication and care that they need ranging from routine immunizations to patient consultations. This position will also serve as a trusted advisor and mentor to our pharmacy technicians, pharmacy interns, front-end associates, and delivery drivers.ResponsibilitiesDispenses prescribed medications and recommends appropriate over-the-counter therapies according to professional standards and state and federal legal requirementsDirects pharmacy staff engaged in counting, filling, packaging, labeling, and delivering medications.Oversees daily ordering and participates in inventory managementReviews prescriptions issued by a physician or other authorized prescriber, to assure accuracy and assess the appropriateness of therapyCertified to provide appropriate immunization services to walk-in patients (includes active CPR certification)Face to face meetings with patients and/or their caregivers to discuss specific pharmacy-related issues such as diabetes medication management, COPD management, supplement use, pain medications, etcAssess patients' needs as it concerns medication adherence.
