Alanté Health is seeking a dedicated Nurse Practitioner to redefine the future of home-based primary care in Arizona. In this hybrid role, the Nurse Practitioner will provide comprehensive patient care through home care and telehealth. We provide Chronic, Principal, Transitional and Wellness Care Management for seniors that improves patient outcomes while significantly reducing hospital readmissions and overall healthcare costs. Our solutions not only saves our patients and their families money, it also solves one of their biggest challenges-transportation. We go to them, whether they are at their own home, in Assisted Living or a Skilled Nursing Facility; we flex to make healthcare easier.

Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition.

Participate in quality assurance and improvement activities.

Provide clinical education and training for patients, families, and facility staff.

Demonstrates knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions.

Travel Requirement

Travel time during the workday is considered part of the role’s responsibilities and will follow all applicable wage and hour guidelines.

Service areas may be adjusted based on patient volume, care coordination needs, and operational demands.

Travel expectations apply to all Nurse Practitioners hired under this updated job description.

Existing Nurse Practitioners will maintain their originally assigned geographic expectations as part of a grandfathered arrangement, unless they voluntarily opt into the updated travel model.

Master’s degree in Nursing or RN license.

Advanced Practice Nursing license.

Fingerprint clearance.

Experience with adult, geriatric populations and population health management preferred.

Strong communication, organizational, and customer service skills.

Knowledge of Skilled Nursing/Transition Rehabilitation Centers (preferred).

Knowledge and use of an Electronic Health Record.

Ability to work with confidential information including HIPAA protected information.

Ability to be proactive and initiate processes with little to no supervision.
