Position Overview:

We are seeking a dedicated and compassionate Bachelor of Social Work (BSW)/ Master of Social Work (MSW) or Licensed Master Social Worker (LMSW) to serve in a multifaceted role combining community outreach, case management, and clinical therapeutic services . This position is ideal for a professional who is passionate about holistic care, advocacy, and empowering individuals and families within the community.

Key Responsibilities:

Serve as a Community Liaison , building and maintaining strong relationships with community partners, agencies, and stakeholders

Conduct outreach to connect individuals and families with appropriate resources and services

Provide comprehensive case management , including assessments, care planning, referrals, and ongoing support

Deliver clinical therapeutic services (individual, family, and/or group counseling) in accordance with scope of practice and licensure

Maintain accurate and timely documentation

Advocate for client needs and support access to social, medical, and behavioral health resources

Collaborate with interdisciplinary teams to ensure coordinated and effective care

Participate in program development and community-based initiatives

Qualifications:

Master’s Degree in Social Work (MSW) from an accredited program

LMSW (or license-eligible) required; supervision available for licensure advancement

Experience in case management, clinical services, or community outreach preferred

Strong interpersonal, communication, and organizational skills

Ability to work independently and as part of a team

Commitment to culturally competent and client-centered care
