Remote Physiatrist

Saint Mary of Nazareth Hospital in Chicago, IL is seeking a compassionate, patient-centered Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based physical therapy evaluation and treatment for a diverse inpatient and outpatient population, including post-surgical, orthopedic, neurological, geriatric, and medically complex patients. You will complete comprehensive assessments, develop individualized plans of care, set functional goals in collaboration with patients and families, and deliver therapeutic interventions that promote mobility, strength, pain reduction, and safe return to daily activities.

Working closely with physicians, nurses, case managers, and other rehabilitation professionals, you will participate in interdisciplinary care planning, provide clear clinical documentation in the electronic medical record, and offer ongoing patient and family education on exercises, precautions, and functional strategies. You will be responsible for conducting gait and balance training, therapeutic exercise, manual therapy, and functional mobility training, as well as recommending and training in the use of assistive devices and durable medical equipment. You will monitor patient progress, adjust treatment plans as needed, and contribute to safe discharge planning and continuity of care.

This full-time position typically involves a hybrid work environment focused primarily on field/on-site clinical care within the hospital and affiliated outpatient settings, with some potential for remote documentation or telehealth follow-ups as appropriate. Flexibility to work some weekend or holiday rotations may be required to support patient needs.
