The facility's layout is designed for easy accessibility, optimizing patient and staff interactions throughout their visit.Key ResponsibilitiesConduct thorough evaluations of patients to assess their physical abilities and limitations, focusing on both subjective complaints and objective findings.Continuously monitor and reassess patient conditions, documenting any changes in progress notes and SOAP (Subjective, Objective, Assessment, Plan) format using our electronic health records system.Provide education to patients and their families about the recovery process, potential challenges, and techniques to promote physical health.Collaborate with multidisciplinary team members to ensure comprehensive care for clients who may require additional services, actively participating in case discussions.Facilitate telehealth consultations as needed, ensuring patients receive appropriate care regardless of their circumstances.Adhere to all organizational policies and minimum utilization goals, contributing to the overall success of the facility.Actively promote a positive workplace culture by embodying the core values of Motion Menders Therapist and serving as an ambassador for excellence in patient care.Patient Population / Client FocusThe patient population at Motion Menders Therapist is incredibly diverse, including children, adults, and older adults, each with various diagnoses and backgrounds.
