RISE provides services for people with disabilities including residential settings, day programs, employment assistance, managed care, and hourly supports.

This encompasses not only individuals and families for whom we provide services and supports, but also employees and stakeholders. Responsibilities Give direct support to people and/or their families as needed in their own homes through the implementation of the RISE Mission, Vision and Values and comply with all regulations and policies related to Home and Community Based Services programs.

Provide assistance to people receiving support through the implementation of the RISE mission, vision, and values.

Promote the rights of people receiving support and contribute to an environment that is free of abuse, neglect and or exploitation.

Assisting people (as applicable to the person) with the following supports: Hygiene

Intimate care

Meal planning/preparation/clean‑up

Accessing their community and providing/arranging transportation

Behavioral supports to include redirecting and/or physically supporting people using approved techniques

Supervision

Medication and medical issues when indicated. Follow medical protocols and doctor’s orders and implement specialized interventions for unique health needs as required.

Provide support in specific service areas as needed: Attendant Care Provider: Support in daily living and hygiene in people’s homes; Habilitation Provider: Provide supports as listed above and also support the achievement of outcomes specified in their plan; Supervises and supports people in their own homes or professional parent homes normally on a daily basis. -$Companionship Home Provider: Provides support to people as listed above and in the achievement of outcomes specified in their plan; Act as an appropriate role model and support people in using appropriate language, dress, table manners, respect, and other behavior as needed.

Respond quickly and effectively to any emergency situations ( e.g. fire, application of First Aid, CPR, approved medical protocols, behavior intervention, etc.).

Follow RISE personnel practices & policies which include (but are not limited to): Obtaining prior approval by following established procedures when time off is desired including reporting to supervisor (per RISE policy) for absence from work

Attending staff meetings and other mandatory training as required by RISE

Follow all RISE and State policies and procedures.

Maintain necessary records, logs, and other documentation as required per ISPP, RISE policy and State of Arizona DES and DDD division requirements.

Qualifications Experience and education in the field of developmental disabilities or a related field preferred.

Desire and interest in working with people with disabilities or special needs.

Background check and Level One Fingerprint Card clearance.

Specific language abilities (other than English) may be required to meet the needs of people/families receiving support and demonstrate a knowledge of and respect for cultural differences.

Reliable transportation to the work site and access to a telephone.

Completion of training courses in CPR, First Aid, and Article 9.

CMS Requirements This caregiver employment position is for parents with children ages birth to 17 years old; once the child turns 18 years of age, parents must sign a new, different job description.

40‑hour per week per minor child limit for parents. This includes assessed hours for both Attendant Care and Habilitation. This limit applies when the minor child is assessed for more than 40 hours of service per week.

Paid caregivers can only provide 16 hours of paid care in a 24‑hour period.

Beginning July 1, 2025, any additional assessed hours for Attendant Care or Habilitation must be completed by an alternate Direct Support Professional. AHCCCS defines a “legally responsible parent” as “Parents who have formal physical and/or legal custody of their minor child including guardians of minor children.”
