Social Services Coordinator
This position is responsible for providing and coordinating social services at the City of Charleston Hope Center, serving as the first point of contact in assessing visitors' social and mental health needs while delivering semi-routine and diversified social work services.
Conduct intake assessments to determine client needs and create individualized housing plans.
Participate in group counseling sessions to ensure proper care delivery.
Maintain data to ensure reporting for the purposes of the community's knowledge and to pursue grant funding in support of services.
Bachelor's Degree with 3 - 5 years of experience or an equivalent combination of education and experience.
Master's degree in social work preferred.
