Home Health Care Registered Nurse Full-time

About This Role The Home Care Registered Nurse provides comprehensive nursing care for patients in their homes, utilizing the nursing process to assess, plan, implement, and evaluate care. This role is ideal for nurses who excel in patient-centered care and collaboration with healthcare teams.
Schedule: Full-Time, Day shift
What You'll Do Conduct patient interviews and explain policies and procedures.
Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status.
Communicate and document patient’s physiologic health status and plan of care.
Assess patient’s psychosocial health status and identify teaching needs.
Coordinate the cost‑effective use of supplies, equipment, and medication.
Implement the plan of care and maintain patient safety precautions.
Participate in planning for discharge and coordinate referrals.
Graduate from an accredited School of Nursing required
Bachelor’s Degree in Nursing preferred
Current License to practice as a Registered Professional Nurse in New York State required
