Advance Care Planning Nurse

Provides direct patient care within nursing standards, focusing on assessment, diagnosis, planning, implementation, and evaluation. Promotes patient and family-centered care, educating on illness management, post-treatment care, and medication use. Ensures patient safety, quality, and regulatory compliance while participating in professional development activities.
Key Responsibilities Develops, implements, and documents individualized care plans in collaboration with the healthcare team, patients, and families.
Communicates care plans effectively across the continuum of care.
Uses critical nursing skills to evaluate physical, psychosocial, emotional, cultural, and spiritual needs, respecting patient preferences and barriers.
Participates in activities that enhance safety, quality, and compliance standards.
Qualifications & Skills Required: Nursing diploma or associate degree, RN license, no prior experience needed.
Bachelor's degree in nursing, less than two years of experience.
